NCR History - Revised 12/06

The Ideal Intervention Paper:
A Protocol for the Consolidation of CPE Learning and a Precursor of Spiritual Care Best Practices

Book of Remembrance:

The History & Research Committee asks that all NCR Supervisors provide them with the information to be kept on file and used to produce your page for the ACPE Book of Remembrance when your days on this earth are done.  These should be submitted to Harvey Berg, 1009 E. Roberta Ave., Waukesha, WI  53186-6713.

You can use the current outline provided by ACPE:    Microsoft Word Format       PDF Format

ACPE policy regarding the Book of Remembrance:  Each deceased supervisor deserves to have a page (double-sided) in the ACPE Book of Remembrance, which is kept at the ACPE Office, with a copy placed in the Book of Remembrance of the ACPE Region to which the person was attached, and a copy placed in the ACPE Archives at the Pitts Theological Library, Emory University, Atlanta, GA.

Place shall be reserved for a passport-sized photo in the upper right hand corner of the front page. The eleven topics listed above should be covered.  The bottom of the second page shall list those writing or consulted about the page, and the date.

While the ultimate responsibility for the B of R pages lies with the Regional Historical Committee/Representative, it is helpful if those closest to a deceased supervisor volunteer to write the page.

It is also suggested that supervisors provide as much of the above information about themselves as possible while they are still living, since this will greatly facilitate things!
